If you’re a resident of Monmouth County or surrounding areas in New Jersey and have Aetna insurance, you might be wondering if this procedure could be covered.

I frequently help patients navigate the insurance process for medically necessary procedures like breast reduction. While every Aetna plan can differ slightly, there are general guidelines they follow to determine coverage. Here’s a breakdown of what Aetna typically requires:

1. Documented Medical Symptoms (Medical Necessity)

First and foremost, Aetna needs to see that the surgery is medically necessary, not purely cosmetic. This means you need to be experiencing significant physical symptoms related to the size of your breasts. Aetna generally requires documentation of at least two of the following symptoms:

  • Headaches
  • Neck Pain
  • Shoulder Pain
  • Back Pain
  • Ulceration or deep grooving from bra straps digging into the shoulders
  • Skin breakdown, irritation, or recurrent infections under the breasts

2. Trial of Conservative Therapy (Showing Other Methods Failed)

Before approving surgery, Aetna wants proof that you’ve tried less invasive treatments without adequate relief. This typically involves a documented 3-month trial of conservative (non-surgical) therapy. Examples include:

  • Physical therapy for associated pain
  • Dermatologic treatment for skin issues (rashes, infections)
  • Pain medication management (prescribed or over-the-counter, documented use)
  • Use of specialized support devices (well-fitting, supportive bras)

You’ll need records showing these treatments were attempted but did not sufficiently resolve your symptoms.

3. Photographic Documentation

Clinical photographs are essential. During your consultation at my West Long Branch office, we will take specific medical photographs. These are required by Aetna to visually assess the situation and are a standard part of the pre-authorization request. Without photos, Aetna will not approve the procedure.

4. Mammogram Requirements

Age-appropriate breast health screening is important. Aetna requires patients over 50 years of age to have a recent mammogram prior to surgery. In my practice, for patient safety and thoroughness, we require a pre-operative mammogram for patients over 35 years old.

5. Minimum Tissue Removal (The Schnur Scale)

Insurance coverage is often tied to the amount of breast tissue the surgeon plans to remove. Aetna uses a calculation based on the Schnur Scale. This scale correlates your Body Surface Area (BSA) – calculated from your height and weight – with a minimum required weight (in grams) of breast tissue that must be removed from each breast to qualify for coverage. During your consultation, we will assess if the amount of tissue likely to be removed meets Aetna’s criteria based on your specific measurements.
